How to Access the Services

Let us know which course of treatment you are interested in and we’d be happy to walk you through the application process.

Behavior Analysis Services

  1. Determine if Applied Behavior Analysis Services are covered by your healthcare insurance or contact our friendly office staff for guidance. Typically carriers will require a prescription from the Doctor for Medically Necessary ABA services. Some Carriers require additional documents and we are happy to assist you through the process.
  2. See our Forms page for an example of what information is required on the letter.
  3. Submit a referral form, consent to treat form and the prescription either via email or through our website.
  4. You will be contacted to initiate the assessment process.

Parent Education Programs

  1. Use the referral form found on our Forms page, fill out the information and submit it.
  2. If you are a case manager referring a patient to receive services under one of our contracts, please be sure to use the appropriate referral system.
